Apply a mid-call checkpoint

Applies one protective utterance or commitment the instant it happens rather than waiting for a terminal disposition, replaying an already-seen eventId with no further effects; every code carries its own typed block except AI_CALL_CONSENT_REVOKED, RECORDING_CONSENT_REVOKED and SELF_HARM_DISTRESS, which must carry none.
